Easy as PIE (Posinflammatory Erythema)SummaryBackground: There is no term to date describing the phenomenon of pink-red discoloration after an inflammatory acne injury. Objectives: to introduce new terminology in the dermatological literature to describe the erythema often seen after the inflammatory acne vulgaris and present a treatment option for this type of erythema. Methods: The new terminology described by erythema after inflammatory acne is addressed, and a treatment option is presented for post-inflammatory erythema. Results: The post-inflammatory erythema is a new precise erythema descriptor that occurs after inflammatory acne. In addition, the laser treatment of pulsed dye improved post-inflammatory erythema in the patients of the authors. Limitations: This document only presents anecdotal cases. Conclusion: The addition of post-inflammatory erythema to dermatological literature can facilitate accurate communication between suppliers and direct laser treatment for post-inflammatory erythema. The authors propose that a new term, post-inflammatory erythema (PIE), be added to dermatological lexicon. In practice, many patients suffering from acne have not only the subsequent effects of scarring (capot, bearing and ice spicing), but also have post-inflammatory dysigment. In darker skin phototypes, this post-inflammatory dispigment often presents as hyperpigmentation. However, in patients with lighter skin types (I-III), post-inflammatory dispigation is often not hyperpigmentation, but instead of discrete erythematous molecules. Acne cannot be the only cause of post-inflammatory erythema, since any inflammatory process of the skin may have residual erythema. On the other hand, erythematothelangictic rosacea is a separate diagnosis, since the redness of rosacea is not part of a resolution process. The post-inflammatory erythema differs from the postoperative erythema described in the literature, since it attributes the erythema to a procedure and is within a surgical scar. PIE refers to the localized skin erythema following any type of skin inflammation, including the erythema that may result in a scar. Consequently, the PIE can reside in the normal skin or inside, while the postoperative erythema is limited to a surgical scar. The exact use of terminology is important to improve communication not only between dermatologists, but also when talking to patients. This is particularly useful as many treatment options are available for acne sequel. Acne can be solved with post-inflammatory hyperpigmentation (HIP), PIE or true scars, which often cause a textured skin change. PIE is different from PIH because PIE describes the residual erythema, while PIH describes the subsequent pigment change. Both the PIH and the PIE tend to resolve over time, so they differ from a true scar that persists. Often, patients will have any combination of PIH, PIE and true scars. Recognition of these distinctions is essential to adequately meet patient concerns about residual acne imperfections. The doctor should ask the patient to identify the lesions of interest for direct treatment towards the PIH, the PIE or true scars, as each has a unique therapeutic approach. The authors present two patients who, after the successful treatment of their facial acne, had residual post-inflammatory erythema. For the treatment of the PIE, the authors used the pulsed dye laser. CASE 1A 30-year-old woman with post-inflammatory erythema secondary to acne was successfully treated with pulsed dye laser (and ). The treatment was given to the face using the pulsed dye laser (Vbeam Perfecta 595nm; Candela SPTL IB, Candela Laser Corp, Wayland, Massachusetts) at a fluency of 6.50J/cm2, pulse duration of 3ms, spot size of 10mm, two passes, for a total of one treatment. The patient was excited about the result on her follow-up appointment a month later. Before the laser treatment of pulsed dye (A). A month after the laser treatment of pulsed dye (B)CASE 2A 16-year-old acne child who was resolved during the treatment of isotretinoin suffered pulsed dye lasers with substantial improvement in the appearance of his post-inflammatory erythema without complications (Figure 3 and 4). The treatment was given in the face using the pulsed dye laser. The adjustment used for the first treatment was a single step to a fluency of 5.75J/cm2, pulse length of 1.5ms and point size of 10mm. The same scenario was used in subsequent visits one and three months later. On each visit, the patient and his father were very happy with the improvement seen after each treatment. Both patients well tolerated the procedure without anesthesia and there were no compLcations. Using the principles of selective phototherolysis, the pulsed dye laser (595nm) is directed to hemoglobin to safely and effectively treat various vascular processes of small diameter, such as vascular capillary malformations, hemangiomas, telangiectas, erythema and angiomas. Large studies have shown minimal side effects including atropical scarring (0.8%), hyperpigmentation (1%), hypopigmentation (2.6%), and dermatitis (2%), among 500 treated patients. One of the authors' patients also received laser treatment while taking isotretinoin without complications. Other lasers and vascular injury devices are available on the market. The experience of the authors has shown that pulsed dye laser treatment is effective in treating patients with post-inflammatory erythema. They propose the addition of post-inflammatory erythema to the lexicon of dermatology to facilitate accurate and effective communication and direct appropriate treatment. Further studies are needed for safety stabLsh, define the optimal laser configuration and quantify the efficacy of pulsed dye laser for post-inflammatory erythema. Microneedling Microneedling is also known as a form of Collagen induction therapy (CIT). By stimulating the production of collagen and elastin with a topical hyaluronic acid or Plasma Rich Protein (PRP), microneedling can rejuvenate the appearance of the skin. This process activates the natural healing and repair systems of the body and can reduce the healing of acne. CO2 Fraccional Laser This laser physically destroys the surface layers of the skin. The recovery process of the wound usually results in the skin that is softer and more regular in the texture. However, this type of laser usually requires anesthesia, significant inactivity time and potential risk of permanent discoloration. Furthermore, patients with darker skin tones are not good candidates for this procedure. Fracture Laser The Fractional Laser Procedure is safe in all types of skin including patients with dark skin tones. Unlike traditional resurfacing lasers, Fractional laser devices allow treatments without the need for anesthesia and inactivity time decrease. Fraction laser therapy offers thousands of deep and microscopic columns of laser energy to your skin. These microthermal treatment zones leave the surrounding skin unaffected. Treatment promotes the process of natural skin healing by producing new collagen and replacing damaged cells. The repaired skin appears healthier, closer and with less surface imperfections. An effective treatment regime typically requires 3 to 5 sessions, spaced at intervals of 2 to 4 weeks. SubcisionThe subcision consists of using a thin pointed instrument to release the adherent scar tissue under a retracted scar. A small amount of blood accumulates under the scar and results in healing of wounds and collagen formation that can help raise depression. Usually, several treatments are required and are combined with some form of resurfacing or fractional laser procedure. Inflammation and brushing may persist for one or two weeks. TCA CrossTCA CROSS is the chemical reconstruction of skin scars (CROSS) using trichloroaceic acid (TCA). The TCA CROSS procedure involves placing small amounts of TCA at high concentration (70-100%) on the surface of the atropical scar. This causes a local inflammatory reaction that leads to the formation of new collagen fibers. Sometimes when acne disappears, it leaves behind a dark place or discoloration area that normally go in pink to red or brown colors. It is caused by a post-inflammatory response of the skin. Post-inflammatory erythema (PIE) and post-inflammatory hyperpigmentation (HIP) are not acne scars. Post-inflammatory erythema (PIE) and hyperpigmentation (HIP) are not exactly the same. Post-inflammatory hyperpigmentation (HPI) is brown or black marks caused by an inflammatory reaction that causes an overproduction of melanine. The causes include getting a grain, aging, pregnancy (melasma), hormones and sun exposure. Post Inflammatory Erythema (PIE) refers to the red or purplish marks that remain behind the acne. The redness is of damage or dilation made to capillaries near the surface of the skin resulting in small flat red marks. Patients may have a combination of PIE and PIH. Treatments Options for PIE and PIH include: Dr Crawford never ceases to amaze me with his tremendous knowledge of any kind of skin problem, but also with his wonderful way to lie down.
